Finding and maintaining suitable accommodation is an important part of living independently. At Issak Disability Care Services, we support participants to build the skills and confidence needed to obtain and sustain safe, stable housing. Our team can assist with tenancy-related goals, including developing independent living skills, understanding tenancy responsibilities, communicating with landlords or housing providers, and connecting with appropriate housing and community supports. We work alongside participants to promote greater independence, stability, and long-term housing outcomes.
Assist-Life Stage, Transition
Life changes can bring new opportunities and challenges. At Issak Disability Care Services, we support participants through important life transitions by helping them build the skills, confidence, and capacity to achieve their goals. Whether transitioning to more independent living, education, employment, or new support arrangements, we work alongside participants to develop practical strategies, strengthen decision-making, and connect them with the services and resources they need to navigate change with confidence and achieve positive, long-term outcomes.
Assist-Personal Activities
At Issak Disability Care Services, we provide respectful and person-centred support with everyday personal activities to help participants live as independently and comfortably as possible. Our experienced support workers can assist with daily personal care, including showering, dressing, grooming, toileting, mobility, eating and drinking, and other activities tailored to each participant’s individual needs, preferences, and goals. We are committed to delivering support with dignity, respect, and a focus on promoting independence and wellbeing.
Assist-Travel/Transport
At Issak Disability Care Services, we support participants to travel safely and confidently so they can access the people, places, and activities that matter most. Whether travelling to appointments, work, education, community activities, or social events, our reliable and person-centred transport supports help participants maintain their independence, build confidence, and stay connected to their community. We tailor our support to each participant’s individual needs, goals, and preferences, promoting greater choice, inclusion, and participation.
Daily Tasks/Shared Living
At Issak Disability Care Services, we support participants living in shared accommodation to develop the skills and confidence needed to manage everyday life. Our person-centred supports include assistance with daily household tasks, personal routines, meal preparation, cleaning, budgeting, and building independent living skills. We work alongside participants to promote choice, responsibility, and positive relationships within the shared living environment, helping them achieve greater independence and an improved quality of life.
Innov Community Participation
At Issak Disability Care Services, we support participants to explore meaningful opportunities that enhance community participation, build confidence, and develop new skills. Through individualised and innovative supports, we help participants pursue their interests, strengthen social connections, and achieve their personal goals in ways that reflect their unique strengths and aspirations. Our focus is on creating engaging opportunities that promote inclusion, independence, and active participation in community life.
Household Tasks
At Issak Disability Care Services, we provide practical support with everyday household tasks to help participants maintain a safe, comfortable, and independent home environment. Our supports are tailored to each participant’s individual needs and may include assistance with cleaning, laundry, meal preparation, shopping, and other essential household activities. We work alongside participants to promote independence, choice, and confidence in managing their home while supporting their health, wellbeing, and quality of life.
Participate Community
At Issak Disability Care Services, we support participants to actively engage with their community, build social connections, and participate in activities that are meaningful to them. Our person-centred supports are designed around each participant’s interests, goals, and preferences, helping them access community, recreational, educational, and social opportunities. We focus on promoting confidence, independence, inclusion, and a sense of belonging, enabling participants to pursue the activities and experiences that are important to them.
Group/Centre Activities
At Issak Disability Care Services, we provide engaging group and centre-based activities that encourage social connection, skill development, and meaningful participation within a supportive environment. Our activities are designed around participants’ interests, abilities, and goals, providing opportunities to build confidence, develop new skills, and connect with others. We support participants to enjoy positive experiences, increase independence, and strengthen their sense of belonging through inclusive and enjoyable activities.
Assistance with Supported Independent Living
Assistance with Supported Independent Living (SIL) helps participants live as independently as possible while receiving the daily support they need in a shared or individual living arrangement. At Issak Disability Care Services, our team provides person-centred assistance with everyday tasks such as personal care, meal preparation, household routines, medication support, building communication and social skills, and developing confidence in daily decision-making. We work closely with each participant, their family, and support network to deliver reliable person-centred support that reflects their individual goals, preferences, and lifestyle, promoting greater independence, safety, and meaningful participation in the community.
